AGNES' GUEST HOME is an assisted living facility located in Galt, CA. Galt is located on U.S. Highway 99, approximately midway between Stockton and Sacramento, CA.

The facility has been upgraded to a Family Style Facility. It has space for 18 residents. The facility is staffed by experienced caregivers 24 hours per day, 7 days per week. Each caregiver is intimately familiar with all residents and the residents' Family.

WHAT IS ASSISTED LIVING PROVIDED BY AGNES' GUEST HOME?
Assisted living facilities are for people needing assistance with Activities of daily living. Usually, assisted living is meant to bridge the gap between independent living and nursing homes. However, Agnes' Guest Homes is licensed to care for residents who need constant care. Assisted living offers help with eating, bathing, dressing, laundry, housekeeping and assistance with medications.

Agnes' Guest Home evaluates each prospective resident to determine the level of care required for that specific patient.

Agnes' Guest Home looks similar to other houses in the neighborhood. When you enter the living room through the front door, you will find it furnished in excellent taste. The couch and chairs are located so that everyone can enjoy watching television.

A Piano is located on the other end of the room for use by Residents and Church groups who visit and sing songs with the residents. The Hallway has two skylights to provide a cheery, well lighted space where residents can sit and talk. The walls are lined with delightful artwork and photographs of life as it was many years ago.

Look inside each of the rooms and you will find them clean and well furnished. Note that none of the rooms look alike. This creates a special homelike atmosphere for each individual Resident. Families bring photographs of the resident and family members to hang on the wall.

Agnes' Guest Home has gone to great lengths to try to make the facility look and feel as much like home as possible. Please feel free to visit the facility at any time.

AGNES GUEST HOMES IS LICENSED TO CARE FOR THE FOLLOWING TYPE RESIDENTS

1. Residents that are not able to live alone but require constant care.
2. Residents that suffer mental cognitive impairment and need help with medications and personal care.
3. Residents who need wheel chairs to move from place to place.
4. Residents who are incontinent and require diapers.
5. Residents who are unable to walk and need help in getting out of bed into wheel chairs.
6. Residents who need help in feeding themselves or who need to be fed by caregivers.
7. Agnes' Guest Home provides respite care for short intervals to provide a break for families taking care of patients at home.

WHAT AMENITIES ARE PROVIDED BY AGNES' GUEST HOME?

1. All medications must have a Doctor's order before assistance with medications can be given to Residents.
2. Medications are centrally stored.
3. A personal medication chart is kept for each resident.
4. Assistance with medication is given (as prescribed by a doctor), by Agnes' Guest Home Staff.

HOW ARE MEDICAL SERVICES SUPPLIED
Agnes' Guest Home has no medical staff on site. Following are ways for residents to get medical services.

1. Traveling Nurses from Lodi Memorial Hospital visit the facility to provide necessary nursing care. Cost of this service is born by the resident or by Medicare or Medicaid if the resident qualifies.
2. Lodi Memorial Hospital has a local care facility in Galt which is staffed by doctors and nurses. The facility is located within walking distance of Agnes' Guest Home.
3. The most convenient Hospital is located in Lodi, CA (approximately 8 miles from Agnes' Guest Home).
4. Other medical facilities and doctors are available in cities from Stockton to Sacramento.

Note: These medical services are the responsibility of the resident or the family of the resident. Agnes' Guest Home Staff will make every effort to contact a family member in case emergency care is required by a resident.

1. Agnes' Guest Home is equipped with sprinklers in case of fire.
2. Exits are clearly marked. There are 4 exits. Front and Back exits have wheel chair ramps. The 2 side exits do not have wheel chair ramps.
3. Exit doors are alarmed but not locked.
4. Grab bars are installed in all bathrooms by the toilet and in the shower.
5. Complete background checks are performed on all prospective employees.
6. Resident's personal property is stored in their individual rooms.
7. Fire Department is only one half mile from Agnes' Guest Home.

1. Each resident can refuse care, meals or activities.
2. Resident rooms do not have locks unless requested.
3. Residents can be signed out of the facility for family visits, lunches, etc.
4. There is No Smoking allowed inside the facility.
5. Special foods can be supplied by resident's family. These foods are the responsibility of the family.

1. Visits are provided by local churches.
2. Exercise ball (Large inflated ball) is tossed between residents (this occurs in the living room).
3. Weekly Bingo Games.
4. Some residents attend a Day Care Center operated by Hutchins Street Square in Lodi. The Day Care Center has various activities. Residents are picked up and returned by the Day Care Center. Lunch is served while at the Day Care Center. There is a charge for this service which is paid for by the resident (at the Day Care Center) or the

5. Some residents may not be physically or mentally able to join into some of the activities. Residents can choose not to participate in any activity.

Nourishing, home cooked meals are available for breakfast, lunch and dinner. Snacks are available in the evening. Usually, all residents eat in the dining room at one time which provides time for eating and socializing. If a resident is not feeling well, staff will serve the meal in the resident's room.

If an emergency arises which requires calling "911," the Resident's Family will be notified immediately. If the resident requires hospitalization, Resident's family may wish to meet the Resident at the Hospital. In any event, the Family of the Resident will, at least, be aware that an emergency exists.

LEVELS OF CARE OFFERED
All levels of care will be considered by Agnes' Guest Home.

AGNES' GUEST HOME/RESIDENT/FAMILY RESPONSIBILITY
Agnes' Guest Home will provide the following for each resident:
All meals, rooms, cleaning of rooms, furniture, laundry service, help with medications, Diaper changing, showers, dressing and any other personal service required to make the resident comfortable.

Family/Resident will provide the following:
1. Pharmacy Care Concepts, located in Stockton, CA offers to package all medications in a bubble pack labeled with the patient's name, day and time medication is required to be taken. The prescription medication is delivered on a weekly basis for each patient. Pharmacy Care Concepts will interface with the resident's Doctor(s) for prescription refills. Most residents subscribe to this service which is very beneficial to busy family members. Pharmacy Care Concepts provides 24 hour service.

3. All special foods or drinks
4. Toothpaste, toothbrushes, lotions, powders, and other personal items.
5. Transportation to and from Doctor or Dentist appointments.
6. All clothing items.

COSTS As with all Assisted Living Facilities, costs are dependent on the level of care required by the resident. After we evaluate the potential resident, we will provide a firm quotation for resident care.
