Intake Information and Inquiry of Services Contact: GGRC Intake Unit
Referral Form: Third Party referrer: (e.g. doctor, nurses)
Intake Form: Parents inquiring for children
Forms should only be used for children below the age of 3 with development delays (e.g. speech).
All other inquiries for persons above the age of 3, email intake@ggrc.org or call 1-888-339-3305
The Prevention Plan (PP) for Golden Gate Regional Center has been received and approved by the California Department of Developmental Services (DDS).
GGRCs 2009 Annual Report addresses 3 focus areas:
Partnership GGRCs efforts to empower individuals, families, service providers, and staff with the information and tools necessary to be successful.
Learning GGRCs efforts to implement innovative strategies and maintain effective stewardship in order to meet the expectations of people served, those providing service, the GGRC Board of Directors and the State of California.
Accountability GGRCs efforts to report on its effectiveness in meeting the expectations of the Board and the State and continually improve performance through quality management. View Report
